Job Overview

Location/Schedule: This position is based at the Kerns building located in NE Portland, OR. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m.

Position Description: Under general direction, the Registered Nurse for Forensic Assertive Community Treatment (FACT) coordinates a wide range of supportive and therapeutic behavioral health services for a caseload of adults with severe mental illnesses and addiction issues with criminal justice involvement while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required that the FACT Registered Nurse (RN) understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. The Registered Nurse assumes responsibility for treatment decisions in keeping with RN licensure provisions and renders care prescribed by physicians and nurse practitioners. Registered Nurse responsibilities shall not be delegated.  

Essential Responsibilities

Provide quality nursing care, including all medical interventions and appropriate documentation. Assist in formulating Nursing Care Plans for all medical/physical conditions. Administer PRN medications based on presenting systems and proper assessment to determine the need for the PRN in collaboration with the LMP. Participate in planning, implementing, evaluating and documenting direct client care activities and medication management, based on sound psychiatric principles. Attend and actively participate in the FACT Treatment Team meetings as scheduled. Act as a liaison with medical staff, pharmacy and laboratory services, and other outside agencies when appropriate to assure continuity of quality client care. Administer psychotropic medications, interpret client response to medication and take appropriate action based on current psycho-pharmacy standards. Collaborate and participate as a team member to aid in establishing medical treatment plans and client care. Interact appropriately with clients, visitors and other community members to support the treatment process, and advise on client rights to treatment, refusal of treatment and involuntary treatment procedures as provided by applicable regulations. Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations.  Maintain accurate and up-to-date documentation as required associated with billing, assessment, treatment, client’s progress and other activities. Attend and actively participate in Nursing meetings and client case conferences. Inform clients about their medications and medical treatments, and provide ongoing support/teaching around medication management for clients and their families.  Call the LMP with laboratory results and any other issues that need to be brought to the LMP’s attention, such as clarification of orders, changes in clients’ mental or medical status, or other information shared by the client that would be helpful for the LMP to know in order to further engage a client in the treatment/recovery process.  Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.  Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients and staff.    Demonstrate responsibility for safety of consumers, staff and property; is familiar with fire regulations and evacuation procedures. Adhere to mandatory abuse reporting laws and HIPAA requirements. 

Role Specific Responsibilities: 

Provide input on all relevant documentation and procedural requirements.  Receive and provide report at FACT Team meetings regarding all client risks and needs. Adhere to all Cascadia medication protocols.  Engage in health promotion, prevention, and education activities. Educate other team members to help them monitor psychiatric symptoms and medication. Transport and/or accompany clients to appointments which connect them to community resources, services and pro-social activities. Provide on-call crisis response and back up interventions and services. With documented consent, engage the client’s family and/or significant others to provide education regarding mental illness and system resources. Assess client behavior and support appropriate service plans. Work with the FACT Counselor III to obtain information to establish the client’s: a) behavioral health needs; b) criminogenic risk and need factors and the required level of cognitive behavioral interventions to mitigate risk; and c) interventions and plan for linkage or reengagement with community resources. Assist in the development of service plans tailored to each individual’s needs. Provide crisis assessment and intervention. Coach clients in setting their own goals and identifying realistic steps in order to achieve their goals. Implement service plans utilizing established treatment models.  Document client contacts, progress and other relevant events and circumstances.  Maintain accurate and up-to-date documentation as required with billing, assessment, treatment, client’s progress and other activities.  Complete electronic medical records (EMR) as required using Cascadia software.   Provide strength based supportive counseling focused on minimizing illness and maximizing client strengths. Participate in evidence based cognitive behavioral interventions for criminal behavior to reduce criminal recidivism and engage in pro-social behaviors in the community. Work with others both inside and outside of Cascadia to assure that assigned clients are receiving the best possible treatment. Adhere to generally accepted treatment standards, departmental rules, company policies and terms and conditions of contracts or grants. 

Secondary Responsibilities:  

Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities. Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings. Able to perform CPR, First Aid, and properly use AED equipment.  CPR/ First Aid certifications are required and must remain current. Perform other duties as assigned. 

Qualifications 

Education: 

An active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification.  Must demonstrate the competencies to become credentialed as a QMHA or QMHP as outlined in OAR 309-016-0005. QMHP: A Bachelors degree in Nursing degree from an accredited college or university.  QMHA: An Associates Degree in Nursing from an accredited college and at least one year of mental health work experience. 

Experience: A minimum of two years of clinical experience providing inpatient or outpatient services is preferred. 

Specialized Knowledge, Skills and Abilities: 

Knowledge of the kinds of medical conditions most commonly encountered by people with a severe and persistent mental illness and an understanding of the physiological basis for the nursing care delivered  Motivational Interviewing  Ability to maintain a trauma informed care environment  Operate Computer hardware and software (Including Electronic Health Record Software) at a level to effectively perform the job functions, including entering and retrieving data. 

Other: Possess a valid driver’s license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course. 

  

Working Conditions 

Work will be performed in a community and/or clinical setting. There is moderate to high risk of personal injury/illness due to violent behavioral, exposure to disease, biohazards, noise and contaminants etc. Work schedule is variable due to the nature of the program and the potential for crisis response and will include rotating on call shifts.   

Mental Demands: The Registered Nurse will have a varied routine which requires the ability to address new and usual circumstances in which outcomes may negatively affect costs, employee morale or clients. There is a degree of unpredictability, disruptions, requiring flexible time management. The position will be responsible for generating revenue by providing a  level of service that meets company or contract standards. 

Physical Demands: A typical work day may consist of sitting, utilizing computers and other office equipment. Work may include transportation out in the community, using agency vehicles in order to support resident access to the community and participation in the community. The Registered Nurse must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations.
