Location/Schedule: This position is based at the Plaza Health Center located in SE Portland, OR, but the majority of work is performed in the community. The schedule is flexible, consisting of a total of 20-hours of work performed (preferably over consecutive days) between Monday and Friday.

Position Description: The Community Peer Provider will work as a member of an interdisciplinary Intensive Case Management (ICM) team to implement and monitor treatment for individuals being served. This position involves participation and implementation of program goals and objectives by providing self-help activities that will promote a strengths-based, wellness-informed, participant centered approach to assisting individuals being served in achieving person-driven goals. The Community Peer Provider will advocate for the individuals and support them to pursue their personal goals and individual wellness journey. As such, the structure and activities promote participant driven goals established on the service plan.
The intensive case management team is a community-based program. The team utilizes a strengths-based approach to providing treatment and care. All staff are mobile, and most services are community based. All team members provide services to a mix of individuals in both Multnomah and Clackamas County. The goal of ICM is to help participants improve their lives, decrease hospitalization, and take charge of their mental health condition.
Essential Responsibilities

Openly identify as a person who has lived the experience of receiving mental health services. Act as a positive role model for individuals being served, showing a recovery and healthful lifestyle orientation. Share own recovery story with individuals being served when appropriate. Provide self-help interventions with individuals in the variety setting as required and directed by the mental health treatment plan. Provide individual and/or group support in living skills, creation of healthful lifestyles, and support in areas such as self-care, holistic wellness, hygiene, community resources, and other life skills as directed by the plan of care and/or goals of this program. May facilitate one or more self-help groups per week. Participate in regular supervisions. PSS will attend clinical case conferences and staff meetings as indicated to gain knowledge, communicate program goals and advocate for the needs of the individuals being served. Comply with and implement the regulations, policies, and procedures which pertain to peer delivered service goals and protocol. Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions. Administer outcome measurement tools with individuals being served and participate in data collection. Reflect positive peer values that include non-hierarchy and mutuality. Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation associated with billing, assessment, treatment, program participant’s progress and other activities as required. Provide input into the therapeutic treatment at the program in collaboration with individual being served and case manager. Actively support team members and other managerial and/or volunteer staff. Act as an advocate for individuals being served, both within the organization and also with other entities. Represent the Peer Wellness Program at community, state, and /or national functions as indicated related to the work of the Peer Wellness Program. May teach participants how to navigate community resources, including but not limited to housing and mass transit. Must document and maintain clinical records in a manner that assures compliance with all agency policy and procedures and local, state and federal regulations through use of Electronic Health Record System (EHR). Adhere to compliance and regulations, required laws and trainings, and the safety of property, staff, and participants, I.e. Mandatory abuse reporting laws and HIPPA, fire regulations, including all company policies and policies under which the facilities is licensed and operates Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings. Complete all annual employee training requirements to stay informed and in compliance and other departmental or program trainings as necessary for professional development Perform other duties as assigned.Qualifications
Education:
A completed Peer Support Training approved by the Oregon Health Authority (OHA). A Traditional Healthcare Worker (THW) certification is required in addition and must be kept current and in compliance with the OAR’s. Must complete a Peer Wellness Training approved by the OHA within two years of the date of employment if incumbent has not completed one.Experience:
Position requires experience as a former or current user of mental health services. Must self-disclose as a current or former user of behavioral health services and must have personal experience with recovery. Some work experience working with adults with severe and persistent mental health challenges is preferred.Specialized Knowledge, Skills and Abilities:

It is preferred that the employee possess a valid driver’s license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
Working Conditions
Mental: Meal periods and breaks are subject to interruption. Universal precautions should be taken due to exposure to disease, biohazards, noise and contaminants
Physical Demands: The nature of the position involves being stationary for consistent periods of time. There is a consistent amount of using a computer and keyboard for documentation, emails and virtual meetings. Mobility to move about the work space and health clinic and sometimes out in the community. Communication and the ability to convey information for therapeutic purposes is extensive, as is the ability to observe and recognize participant behavior that is not always verbalized, recognize participant ques. There may be infrequent use of stairs, stooping, transport 20 to lbs. On occasion may need to run or move quickly to avoid crisis situations
